The name of the deceased, the location, the name of the informant, and a contact telephone number must be provided. Once the notification is received, an agent will attend as soon as possible at the previously indicated home, hospital, care home, or other location.

Medical certificate of decease or urgent care form signed by the doctor

Updated policy and/or latest receipt if they have insurance
In the case of transfer, cremation or judicial death, Grup Muntaner Ribot will provide the necessary documents to be signed by the applicant.
